About the only thing I can suggest is to try again doing exactly what you did - make sure you "safely remove" from the card reader before pulling it out, like with all Windows removable drives/media.
If it consistently fails, I'd think the scanner is at fault and needs replacement, sadly.
Jay, sadly this is what I am coming up with also. Unbelievable.
I now have two separate microSD cards both of which have been formatted apparently correctly.
One thing I discovered: When you put the microSD into the scanner and power it up, it comes up saying SD card file error, use Sentinel blah blah... If you then plug the scanner into the USB port on the PC, the Sentinel software will NOT recognize the scanner. If you pull up devices on Windows, the F drive is listed but when you right click on the BCD436hp icon it says "unable to start (code 10)" on the windows desktop.
Does that give you any clues?
